A student must submit a completed scholarship application, complete contact and academic information page, Essay 1, Essay 2, Essay 3, a volunteer agency certification form, Authorization and Release for Use of Name, Voice, and Likeness, a Suffolk unofficial transcript demonstrating a cumulative GPA of at least 3.0 in college-level coursework, and two letters of recommendation. Requirements vary for new Suffolk students.
